Contract and Disclosure

Client acknowledgment and assumption of risk and full release from liability of LS FITNESS AND BEAUTY LLC. Client Acknowledges any physical activities performed and based on LSFB programming involves the inherent risk of physical injuries or other damages, including, but not limited to only physical injuries – but internal as well. Client agrees to assume all risk and responsibility for not exceeding his or her physical limitations.

LSFB Coaches are not doctors or registered dietitians. The contents of any further documents and/or programs should not be taken as medical advice. It is not intended to diagnose, treat, cure, or prevent any health problem or disease - nor is it intended to replace the advice of a physician. All documents included or exchanged between LSFB and the Client are the intellectual property of LS FITNESS AND BEAUTY LLC. Training Programs, Nutrition Plans, and Individual Coaching Advice are not to be copied, sold or redistributed without our written consent.

EFT Request and Authorization: Client hereby authorizes LS FITNESS AND BEAUTY LLC to make periodic charges of withdrawals (“EFT Authorization”) from the account used to pay the initial payment designated by Client and accepted by LS FITNESS AND BEAUTY LLC, for the payment of any and all fees, late fees, costs, expenses or any other monies due LS FITNESS AND BEAUTY LLC under the terms and conditions of the Agreement. The client may change the account designated, cancel agreement, or request alteration of the initial agreement, with giving 30-day notice prior to the next billing cycle.
